Job Description for Care Manager – Home Care/Elder Care (The Care Manager is the clinical plus operational bridge between family, staff, and Sparsh leadership)

Location: Across Hyderabad-Field plus Office-Hybrid

Job Overview

We are seeking a dedicated and responsible *Care Manager* to oversee and coordinate home care and home nursing services. The ideal candidate should possess strong coordination, communication, and client-management skills to ensure seamless service delivery and high-quality patient care.

Key Responsibilities:

- Client Onboarding: Assess patient needs, home environment, risk factors. Create custom care plan plus budget with family.

- Staffing & Deployment: Match GDA/ANM/GNM/BSc to cases based on skill, language, location. Handle replacements/leaves.

- Clinical Supervision: Daily/weekly audits of care logs, vitals, I/O charts. Ensure SOP compliance for all roles.

- Training & Quality: Conduct Elder Empathy Test plus 1-day onboarding. Run monthly refreshers on fall prevention, pressure sores, dementia care.

- Family Communication: Daily updates to family via call/WhatsApp. Handle complaints, escalation, billing queries.

- Emergency Management: 24x7 on-call. Coordinate ambulance, hospital admission, doctor visits. Maintain ICE contacts.

- Documentation & Compliance: Maintain patient files, INC/BMW/NABH records, incident reports, staff attendance.

- Performance & Growth: Track staff KPIs, do appraisals, identify high-potential GDA→Team Lead pipeline.

Eligibility/Qualification Criteria:

- BSc Nursing / BSW / MHA / MPH preferred. GNM with 5+ yrs exp accepted

- Relevant experience in healthcare, home care, nursing services, or care coordination is preferred.

- Excellent communication, interpersonal, and organizational skills.

- Ability to work in rotational shifts.

- Must possess a valid driving license and be comfortable driving.

- Strong problem-solving skills and a customer-centric approach.

Compensation & Benefits
