Staff Nurse

A Home Health Nurse (Registered Nurse or Licensed Practical Nurse) travels directly to patients' homes to provide one-on-one medical care, manage chronic illnesses, aid post-surgical recovery, and support independent living.

Core Responsibilities

• Clinical Assessments: Perform head-to-toe physical evaluations, monitor vital signs, check wound healing, and track patient progress during each visit.
• Skilled Procedures: Administer physician-prescribed medications, IV therapies, injections, and perform sterile wound care or manage medical equipment (e.g., catheters, feeding tubes). 
• Care Planning & Coordination: Collaborate with physicians, physical therapists, occupational therapists, and social workers to build and update individualised care plans. 
• Patient & Family Education: Teach patients and family caregivers how to manage medications, recognise early warning signs, prevent falls, and perform daily care routines. 
• Documentation & Compliance: Maintain real-time charting and complete standardised clinical assessments (such as OASIS forms for Medicare-certified agencies).
